Your Privacy:  Your privacy is taken seriously.  I collect information from you purely in order for me to fulfill my obligations to you in my role as Registered Chiropractor and an Emmett Practitioner.   

Your contact details:

Your contact details are held on computer software relating purely to appointment schedules for my patients.  This information is not released to third parties.   The use of these details is to make or re-arrange appointments and if needs be to contact you in relation to any alterations.  This information is accessible solely by myself and Reception at Sue Armstrong Consultancy Ltd. 

For some patients I hold contact details on my personal computer and/or mobile devices – these are password/code protected used by and accessed solely by me. Unless specifically requested by you I will not disclose your contact details to any third party.

Your Personal Information:  

Personal information including matters relating to your health and lifestyle which relate to your condition and treatment as my patient will be given to me on the understanding that this information is used by me for the purposes stated.  I maintain paper files which are stored correctly in locked filing cabinets accessible only by myself. The content of your personal file is not divulged to any third party unless expressly requested by the patient.  A signed waiver will usually be obtained in these circumstances.

It is a requirement of my Professional Indemnity Insurers that all files are kept for 7 years after the last treatment.  These files are kept in a locked filing cabinet accessible by myself alone.

The “Lawful basis” on which I record your personal information is “Contract”.  

Your rights with respect to the personal information I hold:

You are entitled to access the personal information I hold on you; any such request should be made in writing please.  If you find inaccuracies please point them out and they will be corrected.  In certain circumstances you may request we erase information I hold on your file but I cannot do so where it will prevent me from meeting my legal and regulatory obligations.

Data Breaches:

In the event that I become aware that information I hold relating to you is lost, stolen, or otherwise breached, and where this constitutes a high risk to your rights and freedoms, I will contact you without delay.
